Add a short decision log beside open questions

The open-questions section works well during discovery. A small decision log next to it would help readers see which questions were settled, when they changed, and what trade-off was accepted.

A date and a one-line reason would be enough. I would also link each decision back to the question it closes so the two sections do not drift apart.

Yes, the link back is important. I do not want a second changelog; just enough context for the next reviewer to understand the current choice.
